What is a mL4L Care Circle?

A Care Circle is made up of friends, relatives, neighbors, and health care professionals who assist a person in monitoring his or her health.

Who can qualify to be a Care Circle member?

Anyone with access to a computer, the internet, and who receives a special invitation from a chronically ill person through myLink4Life.

Can I be in a Care Circle if I live in another state?

What will I do as a Care Circle member?

There are a number of things you can choose:

  • Receive reminders to take readings
  • Remind the person to take a reading or medication
  • Call or visit the person to give them encouragement
  • Receive alerts when readings are outside pre-set limits
  • Help with emergency actions when necessary

How often will I receive messages or alerts?

You will set the frequency and method (email, text message, or phone call). You can coordinate with the healthcare practitioner regarding actions to take in case of an emergency alert.
